Fixed instant publishing bug

We fixed a bug where clicking a link inside the preview iframe in some cases navigated to a malformed edit URL. Instead of opening the page, the editor showed ‘md’ as the title and a stray ‘md:’ field. Flat pages and one-level pages were unaffected, so the bug stayed hidden until a deeply-nested page (two or more directories) was clicked from the preview. This bug also only occured when instant publishing was properly enabled and you were navigating from the preview iframe. In other words: the impact has been very small, but we are glad it is fixed now.
